For physiotherapy and referred swims we require the completion of a Veterinary referral form, for fun and sporting swims we require the completion of a Veterinary consent form. We kindly request that all clients attending for their first hydrotherapy session reads our Terms and Conditions, this form will need to be signed and given in on or before the first session or can be signed on the day of their appointment.
